CDC Patient Safety Structural Measure (PSSM)

The Centers for Medicare and Medicaid Services (CMS) Patient Safety Structural Measure is an attestation-based measure to assess a hospital’s structure and culture in accordance with patient’s safety. The Patient Safety Structural Measure helps to identify hospitals that have a system and culture for patient safety.

IHI National Steering Committee for Patient Safety

The Institute for Healthcare Improvement (IHI) convened the National Steering Committee for Patient Safety (NSC) as a collaboration among 27 national organizations in the US committed to advancing patient safety. IHI charged the NSC with the creation of the first US national action plan for patient safety.

Coomunication and Optimal Resolution (CANDOR) Toolkit

The Communication and Optimal Resolution (CANDOR) process is a process that health care institutions and practitioners can use to respond in a timely, thorough, and just way when unexpected events cause patient harm. Based on expert input and lessons learned from the Agency's $23 million Patient Safety and Medical Liability grant initiative launched in 2009, the CANDOR toolkit was tested and applied in 14 hospitals across three U.S. health systems.
