GUARDIANS of the PSSM

To inspire every hospital to authentically adopt the CMS Patient Safety Structural Measure (PSSM) and uphold a trustworthy Attestation Statement process.

Welcome!

We're ten volunteers who came together to give you the tools to understand hospital patient safety and push for better. We call ourselves the Guardians of the PSSM, a project of Patients for Patient Safety US (PFPS US).

Whether you're advocating for your own safety, a loved one's, an entire facility's, or every patient in the country, this is where you start.

In 2025, CMS adopted the Patient Safety Structural Measure (PSSM), a set of 25 best practices hospitals must now attest to every year, covering leadership commitment, culture, transparency, and how seriously they engage patients and families.

Starting in late 2026, CMS will publish every hospital's score for the public to see. We arm you with the vocabulary to connect what you've experienced to that measure, so it's clear where your hospital is falling short of best practice, regulation, or both, as we all push for change.
